WHAT SHOULD I EXPECT?
You can expect your first visit to last around an hour. Upon arrival you will be greeted by our receptionist, Nicole, who will ask a few questions and help you get started on your paperwork. After your paperwork is complete, you will be brought to an exam room by one of our chiropractic assistants who will get your medical history and review your symptoms and/or injury so the doctor will be most informed when he meets you. Your follow up appointments will be quite a bit shorter because the paperwork and history portion are already done and we can treat you right away.

Insurance/Payment Policies
GFCC will bill any eligible insurance policy as a courtesy. For payment on your account, we accept cash, check and credit payments. We also offer a 15% discount to self pay patients who pay at time of service.
Medical Record Release Authorization
Please download the medical record release form to bring in with you to your visit.
Motor Vehicle Accident
If you are being seen for an injury from an MVA, please bring this form in with you.
Workers Compensation
If you are being seen for an injury from a Workers Comp case, please bring this form in with you.
